Velashape, cellulite buster?

We're looking into Velashape for our plastic surgery practice in Modesto. It seems to be very user dependent and requires some careful patient selection for the cellulite and fat reduction.

We're also looking at Ultrashape as a possibility fo cellulite and fat reduction using focused ultrasound. The ultrashape company is completely separate from the velashape company.

The Velashape company is syneron who also makes lipolite.

prevelle silk from mentor esthetics

We were visited yesterday by our esteemed mentor rep Tom, who brought us lunch from Picasso's Modesto Sandwich Shop (downtown).

We sat as a group and talked about hyaluronic acid fillers and especially about prevelle which is a new filler, recently FDA approved which includes Lidocaine in the mixture. There are several pros and cons to consider. We're going to be carrying this filler (prevelle - silk) as there are many patients whom we feel would benefit.

Firefox, roboforms, waverevenue found on my server in Modesto, ca

Today 6-28-08, I found two new programs on my server:

1. Firefox - which is a browser, like internet explorer.
2. Roboforms - a fill in the blank program, helps to remember passwords.

There's also another thing that's been discovered on my program which is waverevenue.

I'm still exploring what these programs do, and I'm not sure why there are on our Plastic Surgery Server.

icontact vs constant contact vs doing it myself

I have the ability to manage my own mailing list to manage bounces and duplicate emails and such - importing and exporting as well.

I was toying with the idea of making it even easier for email marketing, if I let another company help with templates and archiving newsletters at icontact and constant contact. These are two companies that offer these services for around $15 dollars a month.

The drawback is the limited number of images and templates one can have.

Apple IPHONE 2.0? Just some comments

the Apple iphone 2.0 is about to come out. I currently use the most basic phone - I don't even know how to text message.

As for the Iphone, it has less of a market share than the blackberry. They say that the upgrade uses a even better network from AT&T - miles better...

Rosacea

Often called "adult acne" but potentially far more serious, rosacea is a condition that primarily affects the skin of the face and typically first appears between the ages of 30 and 60 as a transient flushing or blushing on the nose, cheeks, chin or forehead, and is usually discovered at its early and mildest stages. Because the early symptoms often masquerade as a sunburn, rash or allergy, many rosacea sufferers fail to realize they have a condition that can be treated. If left untreated, the disease may eventually progress to inflammatory rosacea, with its papules and pustules. These are small, red, pus-filled bumps that resemble pimples. In some cases, the eyes may also be affected in varying degrees, resulting in discomfort and a bloodshot appearance — sometimes including swollen blood vessels and small, hard bumps on the eyelids. This condition is known as ocular rosacea and could eventually lead to impaired vision if left untreated. In severe cases, the nose may become red and swollen from excess tissue, a condition called rhinophyma, which gave the late comedian W.C. Fields his trademark

Actinic Keratosis

Actinic keratosis is a lesion of the skin – it feels like a dry little patch, but it is actually a pre-cancerous lesion (meaning if untreated it can or will become cancer). If we see Actinic Keratosis we usually recommend removing the lesion or using a topic prescription cream called efudex.

What is Actinic Keratosis (more specifically)?

A skin disease also known as solar keratoses. These lesions are the result of years of unprotected exposure to the sun's ultraviolet (UV) rays. Although you may have been extra careful about sun protection in the recent past, if you spend time in the sun when you were younger, you have put yourself at risk for developing AKs. Many adults have AKs

AKs (actinic keratosis) actually represent the earliest stages of squamous cell carcinoma (SCC). Since the 1960s the incidence of the SCC has been increasing 4%-8% per year. SCC affects about 100,000 Americans each year and 2,000 to 2,500 Americans die each year due to SCC of the skin. At Surgical Artistry, Inc, we hope to take part in reducing this rate of increase, if not halt the increase all together and lead to a decrease in skin cancers.

What is Efudex and how does it work?
AK and sBCC lesions multiply at a faster rate than normal cells. Fluorouracil, the active ingredient in EFUDEX®, is believed to interfere with a cell's ability to reproduce. As a result, the unhealthy cells absorb the fluorouracil in EFUDEX® faster than healthy, normal cells. Efudex requires a prescription and is available at most pharmacies.

It is believe that Efudex lowers the expression of P53.


How is this relevant to our practice - we carry CE Ferulic.

The connection here is P53!
In the latest study (Pinnell) we see that CE Ferulic also prevents and the activation of the P53 gene in live human skin which leads to actinic keratosis (pre-cancers) as well as some forms of skin cancer. CE Ferulic Serum is not only an important cosmetic product, but has also proven to be important for overall skin health. The healthier skin is, the better it looks cosmetically.

Advice to a budding Plastic Surgery PA (Physician Assistant)

My advice to a budding Plastic surgery PA to learn before graduation (whether you want it or not):

Learn lasers, learn injections, learn skin care/facial rejuventation (retin A, Hydroquinone, UVA UVB), learn about spider vein treatments (sclerotherapy and lasers), and of course learn about how to do plastic surgery procedures, especially the ones that have two sides:
Tummy Tuck, Breast Aug, Breast Reduction.
Learn how to sew really nicely and fast.
Your best stitches will be an interrupted deep dermal stitch, and a running subcuticular stitch.
You have to have a goal of sewing better and faster than the surgeon you're watching when you're a student PA.
Learn to wear a headlight.
Learn both instrument and hand ties (for sutures). The instrument tie will be more important for you as a plastic surgery PA. You will eventually tie with the instrument faster than some can tie with their hands.
If you want to do much micro surgery or ocularplastics, you'll need to get used to and buy surgical Loupes (but this is low on the priorities right now).
Learn about the muscles of the face, abdominal wall, and chest wall. Learn about their nerve supply and blood supply. Also learn about the muscles / nerves / blood supply of the upper extremities. Many plastic surgeons are also hand surgeons.

Study: Acupuncture may boost pregnancy

It sounds far-fetched — sticking needles in women to help them become pregnant — but a scientific review suggests that acupuncture might improve the odds of conceiving if done right before or after embryos are placed in the womb.
The surprising finding is far from proven, and there are only theories for how and why acupuncture might work. However, some fertility specialists say they are hopeful that this relatively inexpensive and simple treatment might ultimately prove to be a useful add-on to traditional methods.
"It is being taken more seriously across our specialty," and more doctors are training in it, said Dr. William Gibbons, who runs a fertility clinic in Baton Rouge, La., and is past president of the Society for Assisted Reproductive Technology. "I have not seen proof ... but we wouldn't mind at all" if it turned out to work, he said.
The analysis was led by Eric Manheimer, a researcher at the University of Maryland School of Medicine, and paid for by a federal agency, the National Center for Complementary and Alternative Medicine. Results were published Friday in the British medical journal, BMJ.
Acupuncture involves placing very thin needles at specific points on the body to try to control pain and reduce stress. In fertility treatment, it is thought to increase blood flow to the uterus, relax the cervix and inhibit "fight or flight" stress hormones that can make it tougher for an embryo to implant, Manheimer said.
The analysis pools results from seven studies on 1,366 women in the United States, Germany, Australia and Denmark who are having in vitro fertilization, or IVF. It involves mixing sperm and eggs in a lab dish to create embryos that are placed in the womb.
Women were randomly assigned to receive IVF alone, IVF with acupuncture within a day of embryo transfer, or IVF plus sham acupuncture, in which needles were placed too shallowly or in spots not thought to matter.
Individually, only three of the studies found acupuncture beneficial, three found a trend toward benefit and one found no benefit. When results of these smaller studies were pooled, researchers found that the odds of conceiving went up about 65 percent for women given acupuncture.
Experts warn against focusing on that number, because this type of analysis with pooled results is not proof that acupuncture helps at all, let alone by how much. IVF results in pregnancy about 35 percent of the time. Adding acupuncture might boost that to around 45 percent, the researchers said.
The authors include doctors from the Netherlands and Georgetown University in Washington, D.C. One is an acupuncturist but had no role in any studies that were analyzed.
The American Society for Reproductive Medicine has no policy on acupuncture. "There's been a lot of conflicting research" on its usefulness, said spokeswoman Eleanor Nicoll.
"It looks like, from the body of evidence out there, that some patients benefit," said Dr. James Grifo, head of the infertility program at New York University.
However, Dr. Zev Rosenwaks, director of infertility treatment at New York-Presbyterian Hospital/Weill Cornell Medical Center, said other studies, reported at recent medical meetings and not included in the published analysis, did not find it helped.
"The jury is still out," he said, but added, "It's unlikely that acupuncture does any harm."
Dr. Ann Trevino, a 37-year-old family physician who recently moved to Houston, is pregnant, and a believer. She had three unsuccessful pregnancy attempts with intrauterine insemination before trying acupuncture with IVF at a fertility clinic in San Antonio where she used to live.
"I had been reading about acupuncture, probably like every other patient on the Internet. I was just willing to do anything possible to improve our chances," she said. With acupuncture, "I just felt very warm and relaxed" when the embryos were placed.
Dr. Francisco Arredondo, who runs Reproductive Medicine Associates of Texas where Trevino was treated, said he started offering acupuncture in October, after patients requested it and because some studies suggested it helped.
Acupuncturist Kirsten Karchmer said she places about a dozen needles in the ears, hands, feet, lower legs, abdomen and sometimes the lower back. It costs $500 a month for treatments twice a week, and patients typically go for three months, she said.
IVF costs around $12,000 per attempt, so a treatment that improves its effectiveness might save money in the long run, Manheimer said.

Orchestra's 'Romantic Overtures' impressive

Before opening this weekend's concert, Maestro David Lockington and the Modesto Symphony took a moment to honor Donelle Page, long-time harpist and personnel manager for the symphony who recently passed away. They did so with an unscheduled performance of Ralph Vaughn Williams' "Greensleeves" featuring one of Donelle's students, who performed the prominent harp part in the piece with confidence and elegance. Donelle will be sorely missed by music lovers in our region for a long time to come.
Violinist Lara St. John took the stage next to begin the printed program with a performance of Tchaikovsky's much-loved violin concerto. That Ms. St. John is a soloist of great individuality and fiery technical skill was immediately evident from the opening bars of the first movement. As the piece progressed, however, I began to feel distracted by her apparent need to impose sometimes gimmicky quirks to her interpretation which did not always serve the music well. While her tone could often be rich and beautiful, it was just as often suddenly scraping or whispering. She often traded a sustained musical line in her phrasing for a sudden dropping off of sound in the name of spontaneity, losing in the process a necessary sense of organic architecture.
I can't blame her for wanting to do something different from the crowd of violinists who play this iconic work, but by the end of the first movement I was not convinced that her efforts were producing an entirely sincere or effective musical result. I hoped perhaps the simple melody of the Canzonetta might encourage more linear continuity, and though there were many intimate moments, I still wished for more natural phrasing as a matter of course.
Ms. St. John's approach to the third movement, however, evoked a playfulness and sense of "hide and seek" that worked quite effectively. I had to smile when the concert patron sitting next to me let out a giggle at one point when the violin part was particularly coquettish. Music is ultimately about helping people access and understand human emotion, and undoubtedly, Ms. St. John accomplished that for many in the audience on many occasions throughout the performance, not least with the sheer excitement of her virtuosity.
For me, however, the really impressive aspect of the violin concerto, was the orchestra's unfailingly tasteful sound and appropriate balance. Credit is due both to the players and to Maestro Lockington, who managed nearly flawlessly the soloist's many capricious tempo changes. They provided a reliable foundation for Ms. St. John's more flamboyant and idiosyncratic interpretation.
The orchestra continued to shine in the second half with the Brahms Symphony No. 2. Here the sound was rich and warm, never forced, and evoked the sense of being embraced in a "sonic hug." The tone welled effortlessly from the celli in the famous lyrical theme of the first movement that sounds like the famous lullaby, and invited that sense of semi-sweet melancholy so often present in Brahms. There were many notable solos in the wind parts, particularly Roy Pollock's horn offerings, which warmed the soul. As Romantic symphonies go, the Brahms doesn't rely on the big brass sound that often ramps up the drama, but when called upon, the brass delivered the goods in this performance. The whole finale builds up in one long march to the final trombone chord that our symphony's players nailed dead on for a thrilling conclusion.
The "Romantic Overtones" concert will be performed today at 8 p.m. in the Rogers Theater, Gallo Center for the Arts, 1000 I St., Modesto. Tickets are $30 to $65. 338-2100.

Juvederm vs. Restylane. A question asked by one of our patients.




Is Juvederm safe? I recently saw some commercials on Juvederm. Can you tell me if you offer Juvederm?



We do not have any exclusive contracts with any products that we use in the office. Restylane and Juvederm are the same product, which is hyaluronic acid. Restylane came out first, and had a proven track record to be safe and effective. Juvederm came out recently, within the last few years, and we had attended a seminar on Juvederm when it first came out.

The difference between the two is in the molecular structure of the hyaluronic acid; with Juvederm, it is slightly more “water-thirsty” than Restylane. It therefore appears to have a longer half life and duration of effect.

We wanted to wait and make sure that Juvederm is just as effective and safe as Restylane, and it appears that It is. I have already contacted the Juvederm rep and have ordered some Juvederm for the office, so I will be offering Juvederm in the future in our Modesto Plastic Surgery office.

Social Networking Websites (life beyond MySpace)

I came across a webpage:

as I was contenplating if there was life beyond MySpace.

Surprisingingly on this list I had not heard of Zorpia, Aol People Connection, Hi5, My Yahoo, Tickle, Everyones Connected or Backwash. What surprized me was that MySpace is still #1. I thought there would be a slide down from #1 for Myspace.

Surgical Artistry Academy - One of the many branches of Surgical Artistry.

Surgical Artistry Academy (SAA) strives to become a medical educational powerhouse located in Modesto, CA. Powered by two UC Davis Assistant Professors of Medicine: Dr. Calvin Lee, and Dr. Tammy Wu. Both award winning scientist physicians from Brown University.

Dr. Calvin Lee brings in expertise as a top 0.5% graduate at Brown University, graduating with a degree in Neuroscience and has formal teaching certification from Brown University. As a teaching assistant, he has taught Pscyhoneurobiology and Human Anatomy at Brown University. He has also worked as a hospital X-ray tech, waiter, actor, musician, computer programmer, web designer, and computer consultant. He has soloed with and been concertmaster of orchestra around the United States as a Violinist in the past and now is working on becoming a worthwhile Pianist. He is on the board of directors of two musical organizations in Modesto: Friends of Music (MJC) and Modesto Symphony Orchestra. He has performed with great musicians such as Dave Brubeck (Jazz) and Rachel Barton Pine (Violinist). Medically, he graduated from Brown University undergraduate (He was also accepted to Harvard and Stanford, but went to Brown University) and medical school after 8 years of study. Then he went on to Case Western Reserve in Cleveland for his Surgical Studies, and completed them in Southern Illinois University in Springfield, Illinois. He came to Modesto in 2003, and was appointed assistant professor for UC Davis, teaching surgery to residents in the family practice program. He was awarded community teacher of the year in 2006 for this UC Davis teaching program.

Dr. Tammy Wu graduated as the #1 student from Brown University Medical School. She has expertise in languages as evident in her ability to speak perfect English, Taiwanese, and Mandarin Chinese. She has won awards in her ability to speak French in a high school competition. She also attended Brown University Undergraduate where she worked in the admissions office and participated in FM radio station activities. She graduated with honors with a degree in Biochemistry. After graduation from Brown University Medical School, she went to study at her first choice institution of Southern Illinois University. The university debatedly the best place to learn plastic surgery. Many Chairmen and Plastic Surgery Presidents have graduated form that program.

We give talks around Modesto which help students decide on a medical career and gives a broad overview on Plastic Surgery and General Surgery. We call the program Beyond Nip Tuck.

We still have all those 500-600 mugs that we gave out with the old telephone number. I gave all these mugs to many of the doctors in town. This was the mug with black and burgundy on it and had the cello logo, before we solidified on the woman logo.

Discipline in Music. Goals, and concentration in Music. From EB/AC

First: Think, reflect, and try to understand. THEN play!

Music: It's a hard, good fight. In this profession the more you know and achieve, the higher the mountain gets. There is no end.

Question yourself, your playing… all the time! How's my: Balance? Direction? Shape? Pulse? Sound? Articulation? Breathing? Hand Position?

Go for the subtleties, sophistication, and detail… not cheap thrills!
Your musicality should not be merely accidental or intuitive. How can you be so musical and yet do so many unmusical things?

You are intelligent and talented. That is NOT enough.

Know yourself, your talent, and especially, your weaknesses. Develop your instincts and use your talents.

Be careful not to become a slave of your fingers. Always make sure that your mind controls your fingers and not the other way around. Technique should always be subservient to the musical idea.

Attitude is everything. Yes, the psychological pressure is difficult. But you knew that, and you chose to be here. Give yourself a chance and believe that you can. Otherwise, you will be on stage and think, "MY GOD! WHAT AM I DOING HERE!" There are many psychological factors on performance. Everyone has ghosts to deal with, but the moment you know them, they cease to exist! Try to understand why you feel nervous. Be grateful for life, for the opportunity to play.

Trust what you know. If we get 80% of what we practice when we perform, we are lucky. Don't try to play better than you can (it can happen, but not usually!) and don't allow yourself to play worse than you should!

To check if something is right, try both extremes: right and wrong. You must know wrong to recognize right.

Music and sound are not equal. Don't go for sound. Go for music. Sound is physical and note-by-note. Music is thinking in advance (intellectual) and emotional.

Fight your reflexes and THINK more! Your brain is incredibly lazy when it comes to participation in what you're doing. It is a spectator of the ballet made by your fingers.
Be aware of the relationship between what you want and what you actually are doing.
Anticipate your intentions. Connect all the wires before you play; know exactly what you are going to do.

Use your brain always, not only "on demand."

You cannot become a great musician through money, through miracles, or through your teacher. You could study with Horowitz... you could study with God!... without the work and effort on your part, nothing will happen.

G Spot Amplification
